What services does CPC offer?

How much do CPC's services cos‍‍‍t?

Our services are covered by most major insurance companies, including Medicare Part B, Medicaid and private insurance prov‍‍‍iders. Just as with a regular physician's office, we will file all claims through your insurance company and bill you for anything not covered by insurance. If you have specific questions about your policy, feel free to contact our office.

How often ‍‍‍will you visit?

We develop a care plan specific to you or your family members individual needs. Once your care plan is established we‍‍‍ will develop a schedule for visits to you at your private residence, independent living, assisted living or memory care facility. Based on your progress or any issues that arise your plan can always be modified or adjusted. If you think you need to be seen before your next scheduled visit, please do not hesitate to contact us directly.

What are your ‍‍‍hours?

Our team of providers typically makes house calls between 8 am and 5 pm, Monday-Friday. There is also a nurse practitioner on call 24/7 to answer questions. If you have an urgent need, please cont‍‍‍act the team nurse at 405.603.4660. If at any time you are experiencing a true medical emergency, please contact 911 directly.

Do I n‍‍‍eed a referral to use CPC?

While many patients come to us through physician referrals, they are not required. The only requirement, per insurance, is that you are considered "homebound" for our services to be covered. I'm a patient's fam‍‍‍ily member. Do I need to be at the visits?‍‍‍

We understand that many times a family member is the one in charge of coordinating care for someone in their family. While it isn't required ‍‍‍for you to be present for visits, we are always happy for you to be at our visits. This can be especially helpful during the first visit when we are trying to make sure we have the most accurate patient information possible.‍‍‍
